All information discussed in the process of counseling is held confidential and will not be shared without the client’s knowledge and written permission, except where required by law, which includes a situation when a client;

1. Threatens serious physical harm to either himself / herself or to someone else.
2. Reports physical or sexual child abuse (even if the victim is now an adult), if there is reason to
   believe that the accused abuser is still in a position to abuse other children.
3. Reports serious misconduct of any registered health professional.

AND / OR

4. The client’s clinical file is subpoenaed to court.

All the information gathered and recorded, for the purpose of providing continuity and quality service, is stored securely and remains confidential. The Personal Health Information Protection Act (2004) and the OCSWSSW Code of Ethics govern my record keeping and disclosure policies.
